      <image:title>Meet the Team - SUPPORT COORDINATOR</image:title>
<image:caption>Hannah Sackett Hannah has been part of the Utah DSPD world most of her life personally, and for over 12 years professionally, with younger siblings that receive support from this division. Hannah understands how overwhelming it can feel to navigate through available support as she’s observed and experienced the personal side of care for individuals in group homes, assisted living and nursing facilities as well as in their homes through respite, personal assistance, and supported living models. She’s been employed through the self-administered model as a family member to individuals receiving DSPD support. She's also volunteered, interned, and worked with disability support groups, care facilities, playrooms, with behavior consultants, and at a holistic wellness center for neurodivergent children with behavior concerns. Now she is pleased to be helping others as a Support Coordinator with Evergreen Supports and Advocacy. Hannah earned a bachelor's degree in Family Life with an emphasis on Human Development through Brigham Young University. She is passionate about helping people best utilize the supports available to them while maintaining high quality, personalized care. Hannah is a powerful advocate for her clients and their care teams. In her personal time, Hannah is a licensed massage therapist, providing massage care to her loved ones, is a novice backyard gardener with big landscape dreams, enjoys making food from scratch, spends time with her large family and takes lots of breaks to play fetch with her doodles. 801-609-4237 | hannah@evergreensupports.com</image:caption>

While attending Weber State University she fell in love with the study of Linguistics and the power of communication. She graduated in 2011 with a Bachelor of Arts in Spanish and Latin American Studies. From there she became a liaison between treatment providers and specialty court programs for the Utah State Courts. Soon after she transitioned to working with youth within the court system where she was introduced to case management. The best part of her job was really getting to know her clients and their families while working to identify their individual needs in order to help create a path to success. Tayla is happy to be joining the Evergreen Team and is looking forward to being an advocate for her community and others across the state. She is thrilled to provide you with quality support and advocacy so we can make sure your needs are met! 801-793-4701 | tayla@evergreensupports.com</image:caption>
